PROBLEM TO BE SOLVED: To identify a patient in a dangerous state and especially a patient having an identified disease by taking the information on these patients out of a data base.

SOLUTION: In an estimated health result modeling process 102, a sample group of patient data is received from a disease data base 100 of patients having the specified nature of diseases. In addition, the prescribed statistics or other information are received from a modeling guide line 104. Then a specific estimation model of a specified nature of disease is generated to decide possibility of the worsening of health. In a risk hierarchical process 140, the data proper to every patient which are received from the data base 100 are analyzed to designate a list of patients who have the specified diseases and also have their health states that may possibly be worsened. In an intervention management process 160, a contact is secured with a patient or a doctor and a health management provider to promote the intervention in the medical treatment of the patient.

【課題】 人の健康管理の分野での電子演算処理技術を
利用して病症と病症介入管理と実施すべき危険状態の患
者を識別する。


【解決手段】 コンピュータにて実行する、同定病症を
有するものと診断された患者を識別するシステムと方
法。
患者に関する情報を少なくとも一つの既存データベ
ースから取り出し、同定病症を有するか、又は、それに
かかりそうな患者のグループについて適切な情報を抽出
するべく、所定の基準に基づいてデータベースにおける
患者情報を処理する手段を備えている。
このシステムで
は連合ルールを含む予測モデルを構築する。
