Abstract:
(EN) Medical devices, systems, and methods for catheter-based structural heart therapies, including positioning of prosthetic mitral valves, make use of catheter structures that can flex when advanced over a pre-bent guidewire. Telescoping transseptal access systems use steering segments that are disposed proximal of a relatively rigid catheter segment (the segment optionally supporting a prosthetic valve) by engaging tissue adjacent the right atrium near the proximal end of the valve, and by telescoping a relatively rigid needle guide distally from the valve across the right atrium to engage tissue of the fossa ovalis. Hybrid pull-wire/balloon articulation systems may optionally employ relatively stiff pull-wire articulation within the right atrium, and relatively flexible balloon articulation systems within the left atrium. More generally, hybrid systems may have catheter systems with pullwires or movable sheath, along with fluid drive and robotic control components.
(FR) La présente invention concerne des dispositifs, des systèmes et des méthodes médicaux pour des thérapies cardiaques structurales reposant sur un cathéter, notamment la pose de valvules mitrales prothétiques, faisant appel à des structures de cathéter qui peuvent fléchir lorsqu'elles sont avancées sur un fil guide préalablement incurvé. Des systèmes d'accès transseptal télescopiques utilisent des segments de direction qui sont disposés à proximité d'un segment de cathéter relativement rigide (le segment supportant éventuellement une valvule prothétique) par mise en prise de tissu adjacent à l'atrium droit à proximité de l'extrémité proximale de la valvule, et par télescopage d'un guide d'aiguille relativement rigide de manière distale à partir de la valvule à travers l'atrium droit de sorte à venir en prise avec un tissu de la fosse ovale. Des systèmes hybrides d'articulation à fil de traction/ballonnet peuvent éventuellement utiliser une articulation à fil de traction relativement rigide à l'intérieur de l'atrium droit, et des systèmes d'articulation de ballonnet relativement flexibles à l'intérieur de l'atrium gauche. Plus généralement, les systèmes hybrides peuvent comporter des systèmes de cathéter à fils de traction ou à gaine mobile, conjointement avec des éléments d’entraînement de fluide et de commande robotique.
