Abstract:
(EN) Methods and devices for transvascular prosthetic chordae tendinea implantation are disclosed. A catheter is advanced into the left atrium, through the mitral valve, and into the left ventricle. A ventricular anchor is deployed from the catheter and into a wall of the left ventricle, leaving a ventricular suture attached to the ventricular anchor and extending proximally through the catheter. A leaflet anchor is deployed to secure a mitral valve leaflet to a leaflet suture, with the leaflet suture extending proximally through the catheter. The leaflet suture is secured to the ventricular suture to limit a range of travel of the leaflet in the direction of the left atrium. Also disclosed is an assembled in situ mitral valve leaflet restraint, having a neo papillary muscle and a neo chordae tendinea.
(FR) L'invention concerne des procédés et des dispositifs d'implantation transvasculaire de cordages tendineux prothétiques. Un cathéter est avancé dans l'oreillette gauche, à travers la valvule mitrale, et dans le ventricule gauche. Un ancrage ventriculaire est déployé à partir du cathéter et dans une paroi du ventricule gauche, laissant une suture ventriculaire fixée à l'ancrage ventriculaire et s'étendant de manière proximale à travers le cathéter. Un ancrage de feuillet est déployé pour fixer un feuillet de valvule mitrale à une suture de feuillet, la suture de feuillet s'étendant de façon proximale à travers le cathéter. La suture de feuillet est fixée à la suture ventriculaire pour limiter une plage de déplacement du feuillet dans la direction de l'oreillette gauche. L'invention concerne également un dispositif de retenue de feuillet de valvule mitrale in situ assemblé, ayant un muscle papillaire néo-papillaire et un néo-cordage tendineux.
