WHAT IS CLAIMED IS:

1. A method of bowel care, comprising: repeatedly administering by iontophoresis a therapeutically effective amount of a drug combination comprising neostigmine and glycopyrrolate to a subject having chronic intestinal pseudo-obstruction to relieve chronic constipation, wherein the chronic intestinal pseudo-obstruction is a result of spinal cord injury and the ratio of neostigmine to glycopyrrolate is 2.5:1 to 10:1 by weight.

2. The method of claim 1, wherein the therapeutically effective amount of the drug combination is a ratio of neostigmine to glycopyrrolate of about 5:1 by weight.

3. The method of claim 1, wherein the spinal chord injury results in paraplegia or quadriplegia.

4. The method of claim 1, wherein the neostigmine and glycopyrrolate are administered at about the same time.

5. The method of claim 1, wherein glycopyrrolate is administered about 1 to about 10 minutes after neostigmine.

6. The method of claim 1, wherein the neostigmine and glycopyrrolate are impregnated on separate iontophoresis patches.

7. The method of claim 1, wherein the neostigmine and glycopyrrolate are impregnated on a single iontophoresis patch.

8. The method of claim 1, wherein the chronic administration occurs at least one time per week over a period of at least one month.

9. The method of claim 7, wherein the chronic administration occurs over a period of at least six months.

10. The method of claim 1, wherein the chronic administration occurs at least three times per week over a period of at least one month.

11. A system for initiating a bowel movement, comprising an iontophoresis apparatus having one or more iontophoresis patches comprising a therapeutically effective amount of a drug combination comprising neostigmine and glycopyrrolate and one or more electrodes configured to deliver an electrical current to the skin of a patient proximal to said one or more iontophoresis patches.

12. An apparatus comprising:

a memory communicatively coupled with a processor;

a power source electrically coupled with the memory and the processor;

one or more leads electrically coupled with the power source at a first end, wherein each lead comprises an electrode at a second end, wherein the electrode is configured to deliver an electrical current to the skin of a patient;

one or more iontophoresis patches including a therapeutically effective amount of at least one or more drugs consisting of neostigmine and glycopyrrolate, wherein each iontophoresis patch is paired with a single one of the one or more leads and wherein the the electrical current delivered by the one or more leads drives the therapeutically effective amount of neostigmine and glycopyrrolate across the patient's skin barrier and into the patient's microvascular blood vessels.
