A system (200) which is designed to determine an optimal insulin infusion profile useful as a postprandial blood glucose control for a patient/meal combination is disclosed. The system comprises: a) an input device configured to receive information about a pre-meal measurement of blood glucose concentration, and size and composition of a meal to be consumed; b) a memory which contains additional information about a pre-meal insulin infusion history, a chosen insulin pattern for the postprandial blood glucose control, and a dynamical minimal model having model parameters which are predetermined for the patient/meal combination; c) a processor programmed to use the received information and the additional information to compute automatically the optimal insulin infusion profile by minimizing an objective function, wherein said minimizing of the objective function results in the optimal insulin infusion profile; and d) a display configured to display the optimal insulin infusion profile. Furthermore, a method for the determination of an optimal insulin infusion profile useful as a postprandial blood glucose control for a patient/meal combination is disclosed.

La présente invention concerne un système (200) qui est conçu pour déterminer un profil de perfusion d’insuline optimal utile en tant que contrôle de glycémie postprandiale pour une combinaison patient/repas. Le système comprend : a) un dispositif d’entrée configuré pour recevoir des informations sur une mesure pré-repas de la concentration de glucose dans le sang, et la taille et la composition d’un repas à consommer; b) une mémoire qui contient des informations additionnelles sur l’historique de perfusion d’insuline pré-repas, un profil d’insuline choisi pour le contrôle de la glycémie postprandiale, et un modèle minimal dynamique ayant des paramètres de modèle qui sont prédéterminés pour la combinaison patient/repas; c) un processeur programmé pour utiliser les informations reçues et les informations additionnelles pour calculer automatiquement le profil de perfusion d’insuline optimal en réduisant au minimum une fonction objective, où ladite réduction au minimum de la fonction objective conduit au profil de perfusion d’insuline optimal; et d) un affichage configuré pour afficher le profil de perfusion d’insuline optimal. La présente invention concerne en outre un procédé pour la détermination d’un profil de perfusion d’insuline optimal utile en tant que contrôle de glycémie postprandial pour une combinaison patient/repas.
