Abstract:
(EN) Devices for therapy control based on electromechanical timing provide for detecting electrical activation of a patient's heart, and detecting mechanical cardiac activity resulting from the electrical activation. A timing relationship is determined between the electrical activation and the mechanical activity. A therapy is controlled based on the timing relationship. The therapy may improve intraventricular dyssynchrony of the patient's heart, or treat at least one of diastolic and systolic dysfunction and/or dyssynchrony of the patient's heart, for example. Electrical activation may be detected by sensing delivery of an electrical stimulation pulse to the heart or sensing intrinsic depolarization of the patient's heart. Mechanical activity may be detected by sensing heart sounds, a change in one or more of left ventricular impedance, ventricular pressure, right ventricular pressure, left atrial pressure, right atrial pressure, systemic arterial pressure and pulmonary artery pressure.
(FR) L'invention concerne des dispositifs de contrôle d'une thérapie basée sur une synchronisation électromécanique, qui permettent de détecter une stimulation électrique du coeur d'un patient, et de détecter une activité cardiaque mécanique résultant de cette stimulation électrique. Une relation de synchronisation est déterminée entre la stimulation électrique et l'activité mécanique. Une thérapie est contrôlée sur la base de la relation de synchronisation. La thérapie peut améliorer la dyssynchronie intraventriculaire du coeur du patient, ou traiter au moins un dysfunctionnement diastolique ou systolique et/ou un dysfunctionnement du coeur du patient, par exemple. Une stimulation électrique peut être détectée par détection de l'application d'une impulsion de stimulation électrique sur le coeur, ou par détection d'une dépolarisation intrinsèque du coeur du patient. Une activité mécanique peut être détectée par détection des bruits cardiaques, d'une modification de l'impédance du ventricule gauche, de la pression ventriculaire, de la pression du ventricule droit, de la pression de l'oreillette gauche, de la pression de l'oreillette droite, de la pression artérielle systémique, de la pression artérielle pulmonaire, ou d'une combinaison de plusieurs desdits éléments.
