Abstract
(EN) An intravascular catheter device having a catheter and an ultrasound ablation manifold is proposed for generating cavitations and acoustic jet streams in the blood to ablate atheroma. The ablation manifold has a power transducer and a coupled leaky acoustic cavity. The leaky acoustic cavity contains a first portion of ultrasonic power emission for intra-cavity ablation while allowing a second portion of ultrasonic power emission to leak outside for an extra-cavity ablation. The power transducer and the leaky acoustic cavity are configured to form a confocal resonant cavity. An intervening bird cage is coupled to the resonant cavity for stronger resonance while maintaining ultrasound leakage. A protective shield is mounted around the waist of bird cage to strengthen resonance and to prevent damaging the intima from an otherwise normal incidence of high intensity ultrasound. A microbubble releasing device and micro pump are also included to further increase the ablation efficacy.
(FR) L'invention concerne un dispositif cathéter intravasculaire comportant un cathéter et une tubulure d'ablation ultrasonique, et permettant de générer des cavités et des courants jets acoustiques dans le sang pour l'ablation de l'athérome. La tubulure d'ablation a un transducteur de puissance et une cavité acoustique de fuite couplée. La cavité acoustique de fuite contient une première portion d'émission de puissance acoustique pour une ablation intra-cavité tout en permettant à une deuxième portion d'émission de puissance acoustique de s'échapper à l'extérieur pour une ablation extra-cavité. Le transducteur de puissance et la cavité acoustique de fuite sont configurés pour former une cavité résonante confocale. Une cage de transport intermédiaire est couplée à la cavité résonante pour une résonance plus importante tout en conservant la fuite ultrasonique. Un écran protecteur est monté autour de la ceinture de la cage de transport pour renforcer la résonance et empêcher tout endommagement de la intima par une incidence d'un ultrason haute intensité, par ailleurs normale. L'invention concerne également un dispositif de délivrance de microbulles et une micro-pompe permettant d'améliorer l'efficacité d'ablation.
