Apparatus and methods adapted for $i(in vivo) chronic measurement of cardiac monophasic action potentials (MAPs). The methods include, inter alia, providing a sensing electrode (22) in contact with cardiac tissue (24) and a reference electrode (26) in proximity to the sensing electrode (22), intermittently inducing a transient localized depolarization or transient injury-like electrical currents in at least some of the cells of the cardiac tissue (24) underlying or adjacent to the sensing electrode (22) and measuring the potential difference between the sensing electrode (22) and the reference electrode (26) during at least part of the duration of the depolarization or injury-like currents. Methods for inducing localized depolarization or injury-like currents in the tissue include, $i(inter alia), localized membrane electroporation, localized electrostatic production of depolarization using voltage clamp to produce a signal representative of MAPs, localized tissue heating, localized application of ultrasound, localized irradiation with light and chemically induced localized tissue depolarization. The apparatus and methods may also be applied for non-chronic measurement of cardiac monophasic action potentials and for measurement of monophasic action potentials from other non-cardiac excitable tissues.

L'invention concerne un appareil et des procédés adaptés pour effectuer $i(in vivo) une mesure chronique de potentiels d'action monophasiques cardiaques (MAP). Les procédés consistent, entre autres, à placer une électrode de détection (22) au contact d'un tissu cardiaque (24), et une électrode de référence (26) à proximité de l'électrode de détection (22). Les procédés consistent ensuite à induire par intermittence une dépolarisation localisée provisoire ou des courants électriques provisoires susceptibles d'être nuisibles dans au moins quelques-unes des cellules du tissu cardiaque (24) sous-jacent ou adjacent à l'électrode de détection (22), puis à mesurer la différence de potentiel entre l'électrode de détection (22) et l'électrode de référence (26) pendant au moins une partie du temps de dépolarisation ou d'activité des courants susceptibles d'être nuisibles. Des procédés d'induction d'une dépolarisation localisée ou de courants susceptibles d'être nuisibles dans le tissu incluent, entre autres, une électroporation membranaire localisée, une production électrostatique localisée d'une dépolarisation mettant en oeuvre un circuit de blocage de tension pour produire un signal indicatif du chauffage localisé du tissu dans les MAP, une application localisée d'ultrasons, une irradiation localisée avec une lumière, et une dépolarisation localisée du tissu par des procédés chimiques. L'appareil et les procédés de l'invention peuvent également s'utiliser pour une mesure non chronique de potentiels cardiaques à partir d'autres tissus non cardiaques sensibles à une excitation.
