Protocols for antitachycardial pacing including biphasic stimulation administered at, or just above, the diastolic depolarization the shold potential; biphasic or conventional stimulation initiated at, or just above, the diastolic depolarization threshold potential, reduced, upon capture, to below threshold; and biphasic or conventional stimulation administered at a level set just below the diastolic depolarization threshold potential. These protocols result in reliable cardiac capture with a lower stimulation level, thereby causing less damage to the heart, extending battery life, causing less pain to the patient and having greater therapeutic effectiveness. In those protocols using biphasic cardiac pacing, a first and second stimulation phase is administered. The first stimulation phase has a predefined polarity, amplitude and duration. The second stimulation phase also has a predefined polarity, amplitude and duration. The two phases are applied sequentially. Contrary to current trought, anodal stimulation is first applied and followed by cathodal stimulation. In this fashion, pulse conduction through the cardiac muscle is improved together with the increase in contractility.

Protocoles de stimulation antitachycardique, qui comprennent une stimulation biphasique administrée au niveau ou juste au-dessus du potentiel seuil de dépolarisation diastolique; une stimulation biphasique ou classique débutant au niveau ou juste au-dessus du potentiel seuil de dépolarisation diastolique, puis diminuant, après capture, jusqu'au-dessous dudit seuil; et une stimulation biphasique ou classique administrée à un niveau situé juste au-dessous du potentiel seuil de dépolarisation diastolique. Ces protocoles assurent une capture cardiaque fiable malgré un niveau de stimulation relativement faible. De ce fait, ils provoquent moins de lésions cardiaques et allongent la durée de vie de la pile, et ils sont moins douloureux pour le patient et plus efficaces sur le plan thérapeutique. Dans les protocoles faisant appel à une stimulation biphasique, on administre une première et une seconde phase de stimulation, chacune ayant une polarité, une amplitude et une durée prédéfinies. Les deux phases sont appliquées successivement. Contrairement à l'idée courante, la stimulation anodique est appliquée en premier, suivie de la stimulation cathodique. De cette manière, la conduction de l'impulsion à travers le muscle cardiaque est améliorée et la contractilité accrue.
