Abstract:
(EN) This invention is a system and method for percutaneous myocardial revascularization using laser energy emitted from the distal end of a catheter. A technique for controlling beam direction allows the creation of channels in a patient's heart tissue with the channels having different orientations relative to the distal end. An arrangement provides for the creation of a plurality of channels at one time by simultaneous application of a plurality of beams of laser energy. A navigation arrangement uses two non-coplanar magnetic sensing coils (124F, 124T, 124S) in the distal end of the catheter cooperating with three sets of three magnetic field generating external coils (i.e., external to the patient) (120F1-120F3, 120S1-120S3, 120T1-120T3). Each of the three sets is sequentially energized and the sensing coils in the distal end sense the magnetic fields established. A computer compares the sensed fields with the known sequence of energizing the external coils and thereby determines the opposition of the distal end. A pressure sensor in the catheter senses ventricular pressure in the patient's heart during and immediately after revascularization.
(FR) Cette invention se rapporte à un système et à un procédé de revascularisation percutanée de l'endocarde et du myocarde utilisant l'énergie d'une lumière laser émise à partir de l'extrémité distale d'un cathéter. Un procédé de commande de la direction du faisceau permet la création, dans le tissu cardiaque du patient, de canaux qui ont des orientations différentes par rapport à ladite extrémité distale. Un agencement assure la création d'une pluralité de canaux en une seule opération, par application simultanée d'une pluralité de faisceaux d'énergie laser. Un agencement de commande des déplacements utilise deux bobines de détection magnétique non coplanaires (124F, 124T, 124S), situées au niveau de l'extrémité distale du cathéter, qui coopèrent avec trois ensembles de bobines externes (c'est à dire externe au patient), génératrices de champ magnétique (120F1-120F3, 120S1-120S3, 120T1-120T3). Chacun de ces trois ensembles est alimenté en séquence et les bobines de détection, disposées au niveau de l'extrémité distale, détectent les champs magnétiques établis. Un ordinateur compare les champs détectés à la séquence connue d'alimentation en énergie des bobines externes et détermine de cette manière l'opposition de l'extrémité distale. Un capteur de pression disposé dans le cathéter détecte la pression ventriculaire dans le coeur du patient, pendant et immédiatement après la revascularisation.
