Abstract
(EN)
The invention provides a system and method for manipulating a tissue structure within a body cavity. In a preferred embodiment, the invention provides a system and method for retracting and supporting the heart wall to provide access into the heart during a cardiac surgical procedure. The system comprises a tissue supporting member (500) positionable through a first percutaneous intercostal penetration into the thoracic cavity. The tissue supporting member has a contact surface (502) configured for supporting a portion of the heart wall. A retractor (40a) includes a shaft (400) with a proximal end, a distal end configured for introduction through a second percutaneous penetration and a diameter less than the width and length of the contact surface. A hook (428) is slidably coupled to the distal end of the shaft for releasably holding the tissue supporting member such that the contact surface is arranged transversely to the longitudinal axis of the shaft. With this configuration, the shaft and tissue supporting member can be introduced through two separate percutaneous penetrations and connected together within the thoracic cavity for retraction of the heart wall.

L'invention se rapporte à un système et à un procédé de manipulation d'une structure tissulaire dans une cavité du corps. Selon un mode préféré de réalisation, l'invention concerne un système et un procédé permettant d'écarter des bords de la paroi du coeur et de les maintenir écartés afin de faciliter l'accès au coeur au cours d'une opération du coeur. Ce système comprend un élément (500) de soutien des tissus, que l'on positionne à travers un premier passage intercostal et percutané dans la cage thoracique. Cet élément comporte une surface de contact (502) configurée pour soutenir une partie de la paroi du coeur. Un rétracteur (40a) comprend une tige (400) présentant une extrémité proximale, une extrémité distale configurée pour être introduite par un second passage percutané, dont le diamètre est inférieur à la largeur et à la longueur de ladite surface de contact. Un crochet (428) est accouplé coulissant à l'extrémité distale de la tige afin de retenir de manière libérable l'élément de soutien des tissus de sorte que la surface de contact soit disposée transversalement par rapport à l'axe longitudinal de la tige. Une telle configuration permet d'introduire la tige et l'élément de soutien des tissus à travers deux passages percutanés séparés, après quoi ils sont accouplés à l'intérieur de la cage thoracique afin d'écarter les bords de la paroi du coeur.
