A method and apparatus for discriminating between stable and unstable ventricular tachycardias based on a measurement of oxygen saturation. Reference values for average oxygen saturation level and of the pulsatile characteristic of the oxygen saturation level are taken while a patient is in a resting condition. Measurements of oxygen saturation are also taken in response to the detection of a high heart rate, and are compared to the reference measurements to discriminate between stable and unstable ventricular tachyarrhythmias. An unstable tachyarrhythmia is diagnosed if a tachycardia is diagnosed and the current oxygen saturation level is similar to the reference average oxygen saturation level or if the oxygen saturation as presently measured displays a decreased pulsatile characteristic as compared to the reference measurements. The discriminator is intended for use in conjunction with or as part of an automated cardioverter of the type capable of delivering differing therapies for termination of stable and unstable ventricular tachyarrhythmias, such as antitachycardia pacing, cardioversion and defibrillation.

Procédé et appareil permettant de distinguer les tachycardies ventriculaires stables et instables, fondés sur une mesure de la saturation en oxygène. On prend des valeurs de référence du niveau moyen de saturation en oxygène et de la caractéristique pulsatile du niveau de saturation en oxygène lorsque le patient est au repos. Des mesures de la saturation en oxygène sont également effectuées en réaction à la détection d'un rythme cardiaque élevé. Ces mesures sont comparées aux mesures de référence en vue de distinguer les tachyarythmies ventriculaires stables et instables. On est en présence d'une tachyarythmie instable si l'on diagnostique une tachycardie tandis que le niveau présent de saturation en oxygène est similaire au niveau de saturation en oxygène moyen de référence ou si la saturation en oxygène telle qu'elle est mesurée sur le champ présente une caractéristique pulsatile diminuée par comparaison aux mesures de référence. Ledit dispositif de distinction est conçu por être utilisé conjointement avec ou en tant que partie d'un défibrillateur automatisé du type capable d'administrer diverses thérapies destinées à supprimer les tachyarythmies ventriculaires stables et instables, telles que la stimulation antitachycardique, la cardioversion et la défibrillation.
