Abstract:
(EN) An artificial implant (20) includes an elongated stem member (24) which is intended for cementless fixation in the medullary canal of a logn bone. The stem member (24) is provided with one or more longitudinally extending channels (36). A porous medium (44) for enabling and encouraging bone ingrowth fixation is bonded to the bottom surface (38) of each channel (36) but is free of the sidewalls (40, 42) of the channel (36). It has a first outer surface (48) lying in a plane generally parallel to that of the external surface of the stem member (24) and second (50) and third (52) surfaces lying generally, respectively, in angularly spaced radial planes containing the longitudinal axis of the stem member (24). The resulting implant (20) has a flexibility which can be tailored to that of the bone. Also, the porous medium enables an initial interference fit with the medullary canal of the long bone as well as bone ingrowth fixation over a substantially enlarged surface area than previously possible. Also, notching the high stress region of the external surface (28) of the stem member (24) as previously caused by application of the porous medium to the outermost, external, surface (28) of the stem member (24) is avoided.
(FR) Un implant artificiel (20) comprend un élément tige allongé (24) destiné à être fixé sans ciment dans le canal médullaire d'un os long. L'élément tige (24) est pourvu d'un canal ou de plusieurs canaux (36) qui s'étendent de manière longitudinale. Un milieu poreux (44), destiné à aider et à encourager la fixation de la croissance osseuse interne, est collé à la surface inférieure (38) de chaque canal (36) mais sans qu'il ne touche les parois latérales (40, 42) de chaque canal (36). Ce milieu comporte une première surface intérieure (48) orientée en un plan généralement parallèle au plan de la surface extérieure de l'élément tige (24) et une seconde (5) et troisième (52) surfaces orientées respectivement, de manière générale, en des plans radiaux à espacement angulaire et contenant l'axe longitudinal de l'élément tige (24). L'implant (20) qui en résulte possède une flexibilité qui peut être ajustée à celle de l'os. De plus, le milieu poreux permet un ajustement initial serré dans le canal médullaire de l'os long aussi bien que la fixation de la croissance osseuse sur une surface sensiblement augmentée par rapport à ce qui était antérieurement possible. En outre, des entailles précédemment produites sur la zone de haute tension de la surface extérieure (28) de l'élément tige (24) par l'application du milieu poreux à la surface extérieure (28) de l'élément tige (28) sont évitées.
