Abstract
(EN)
A laser therapy system (10) having improved effectiveness in surgical use, particularly in laser angioplasty, can be formed by employing a pulsed source of radiation. The pulsed energy source is preferably an excimer laser (14) having a coherent beam of ultraviolet radiation and preferably is employed in conjunction with a dye laser (20) to produce an output beam which is tunable over a wide portion of the ultraviolet and visible spectrum. The output beam is coupled via a novel coupling device to an optical fiber (48) disposed within a surgical instrument, for example, a percutaneous catheter (80). The operative components are sealed in gas-tight, liquid-tight housing (12). An electronic controller (46) monitors and adjusts the output energy density, duration, and pulse repetition rate. In operation, a pulsed, high energy beam of extremely short duration is available to remove atherosclerotic plaque with less damage to the underlying tissue and less chance of perforating the blood vessel wall.

Un systéme de thérapie au laser (10) à efficacité améliorée pour des utilisations chirurgicales, en particulier pour l'angioplastie au laser, peut-être formé en utilisant une source pulsée de rayonnements. La source pulsée de rayonnements est de préférence un laser "excimer" (dimer exité) (14) ayant un faisceau cohérent de rayonnement ultra-violet et associé de préférence à un laser à colorant (20) pour produire un faisceau de sortie syntonisable dans une grande partie du spectre ultra-violet et visible. Le faisceau de sortie est couplé via un dispositif novateur de couplage à une fibre optique (48) agencée dans un instrument chirurgical, un cathéter percutané (80) par exemple. Les composants opérationnels sont hermétiquement enfermés dans un boîtier (12) imperméable aux liquides et aux gaz. Une commande électronique (46) contrôle et ajuste la densité, la durée et la vitesse de répétition des impulsions de l'énergie de sortie. Pendant son utilisation, le systéme produit un faisceau pulsé de haute énergie de durée extrêmement courte pour enlever des plaques artérioscléreuses en causant moins de dommages au tissu sous-jacent et avec moins de risques de perforation de la paroi du vaisseau sanguin.
