CLAIMS
LUBRICATION BETWEEN CONTACTING SURFACES
1. A breast implant system comprising at least one casing (3, 4) with a flexible outer shape for implantation in a patient's body so as to form part of a breast implant (10) and further comprising at least one first element (1) contained in the casing and optionally at least one second element (2) also contained in the casing, characterized by a reservoir (R; R', R", R") comprising a lubricating fluid and connected to the casing (3, 4) so as to allow lubricating fluid to be supplied to and removed from the casing in order to reduce surface friction between an outer surface of said at least one
first element (1) and surfaces contacting said outer surface.
2. The breast implant system of claim 1, wherein the lubricating fluid is a liquid or a gel.
3. The breast implant system of claim 2, wherein the lubricating liquid or gel is isotonic.
4. The breast implant system of any of claims 1 to 3, adapted to change the friction between surfaces in the breast implant to allow a change of the outer shape of the breast implant post-operatively.
5. The breast implant system of claim 4, adapted to change the friction between surfaces in the breast implant to allow a change of the outer shape of the breast implant non-invasively.
6. The breast implant system of any of claims 1 to 5, wherein the outer surface of the at least one first element (1) is displaceable relative to a contacting
inner surface of the casing.
7. The breast implant system of any of claims 1 to 5, wherein the outer surface of the at least one first element (1) is displaceable relative to a contacting
outer surface of the at least one second element (2).
8. The breast implant system of any of claims 1 to 5, wherein the outer surface of the at least one first element (1) is displaceable relative to a contacting
inner surface of the casing and relative to a contacting outer surface of the at least one second element (2).
9. The breast implant system of claim 8, wherein the outer surface of the at least one second element (2) is displaceable relative to a contacting inner surface of the casing.
10. The breast implant system of any of claims 1 to 5, wherein the outer surface of at least two first elements (1) is displaceable relative to a contacting inner surface of the casing.
11. The breast implant system of claim 10, wherein the outer surface of the at
least two first elements (1) is displaceable relative to a contacting outer surface of the at least one second element (2).
12. The breast implant system of claim 10, wherein the outer surface of the at
least two first elements (1) is displaceable relative to a contacting inner surface of the casing and relative to a contacting outer surface of the at least one second element (2).
13. The breast implant system of claim 12, wherein the outer surface of the at
least one second element (1) is displaceable relative to a contacting inner surface of the casing.
14. The breast implant system of any of claims 1 to 5, wherein the outer surface of at least three first elements (1) is displaceable relative to a contacting inner surface of the casing.
15. The breast implant system of claim 14, wherein the outer surface of the at
least three first elements (1) is displaceable relative to a contacting outer surface of at least one second element (2).
16. The breast implant system of claim 14, wherein the outer surface of the at
least three first elements (1) is displaceable relative to a contacting inner surface of the casing and relative to a contacting outer surface of at least second element (2).
LOCATION OF RESERVOIR
17. The breast implant system of any of claims 1 to 16, wherein the reservoir (R;
R', R", R```) is adapted for implantation inside the patient's chest area.
18. The breast implant system of claim 17, wherein the reservoir adapted for implantation in the patient's chest area is adapted for implantation outside the thorax.
19. The breast implant system of claim 18, wherein the reservoir adapted for implantation outside the thorax is adapted for implantation below the minor pectoralis muscle (40).
20. The breast implant system of claim 18, wherein the reservoir adapted for implantation outside the thorax is adapted for implantation between the major and the minor pectoralis muscles (40, 41).
21. The breast implant system of any of claims 1 to 17, wherein the reservoir is adapted for implantation within the patient's body remote from the breast implant (10), the system further comprising at least one conduit (5) between the remotely implantable reservoir and the casing for fluid exchange between the reservoir and the casing.
22. The breast implant system of claim 21, wherein at least part (Hu) of the reservoir (R) is adapted to be implanted in the armpit or underneath the patient's arm.
23. The breast implant system of claim 21, wherein at least part of the reservoir is adapted for implantation in the patient's abdominal cavity.
SUBCUTANEOUS IMPLANTATION OF RESERVOIR / MANUAL OPERATION
24. The breast implant system of any of claims 21 to 23, wherein at least part
(R'") of the reservoir (R) is adapted to be implanted subcutaneously such that
fluid can be exchanged between the casing and the reservoir by manually compressing the subcutaneously implanted reservoir (R) or part (R") thereof, thereby urging fluid to flow from the reservoir into the casing.
25. The breast implant system of claim 24, comprising at least one valve (28) between the reservoir and the casing.
26. The breast implant system of claim 25, wherein the at least one valve (28)
includes a pressure relief valve which opens in at least one direction at a predetermined pressure.
CONSTITUTION AND CONTENT OF ELEMENTS
27. The breast implant system of any of claims 1 to 26, comprising 10 or more of the first elements (1) contained in the casing (3, 4).
28. The breast implant system of claim 27, comprising 100 or more of the first
elements (1) contained in the casing (3, 4).
29. The breast implant system of any of claims 1 to 26, wherein the first element (1) forms a cushion or pad containing a filling material and having a flexible
outer wall forming said outer surface.
30. The breast implant system of any of claims 1 to 27, wherein at least one of the outer wall of the first element (1) and the outer wall of the casing is flexible.
31. The breast implant system of claim 28, wherein said at least one of the outer wall of the first element (1) and the outer wall of the casing is elastic.
32. The breast implant system of any of claims 1 to 31, wherein the outer wall of the casing has a shape adapted to allow lengthening of the functional length thereof without interfering with fibrosis covering said outer wall, when the breast implant is implanted in the body..
33. The breast implant system of claim 32, wherein the outer wall has at least
one wrinkle or cease adapted to allow lengthening of the outer wall without lengthening fibrosis covering the outer wall when implanted.
34. The breast implant system of any of claims 1 to 26, wherein the first and second elements (1, 2) each form a cushion or pad containing a filling material and having a flexible outer wall forming said outer surfaces.
35. The breast implant system of any of claims 29 to 34, wherein the flexible outer wall is slack.
36. The breast implant system of any of claims 29 or 34, wherein the filling material is a fluid.
37. The breast implant system of claim 36, wherein the filling material comprises at least one material of the following group of materials: gas, liquid, gel, foam or any other flowable material, or a combination thereof.
38. The breast implant system of claim 37, wherein the fluid is a silicone liquid or a silicone gel.
39. The breast implant system of claim 38, wherein the fluid is a gas.
40. The breast implant system of any of claims 29 or 34, wherein the filling material is granular.
CONTENT CHANGE OF ELEMENTS
41. The breast implant system of any of claims 1 to 39, wherein at least one first element (1) and at least one second element (2) within the casing are interconnected so as to allow for exchanging content between them.
42. The breast implant system of claim 41, wherein fluid can be exchanged between the first and second elements (1, 2) by manually compressing the one or the other, thereby urging fluid to flow from the one element into the other element.
43. The breast implant system of claim 42, comprising at least one pressure relief valve (12) which opens at a predetermined pressure.
44. The breast implant system of claim 43, wherein the pressure relief valve (12) is a two-way pressure relief valve which opens in the one or the other direction depending upon the side where the predetermined pressure is applied.
45. The breast implant system of claim 41, further comprising at least one intermediate reservoir (7) in fluid connection with both the first element and
the second element such that fluid can be exchanged between the intermediate reservoir (7) and the first and second elements (1, 2) so as to change their respective content.
46. The breast implant system of any of 'claims 1 to 39, further comprising at
least one further reservoir (R1, R2) in fluid connection with at least the first element (1) such that fluid can be exchanged between the further reservoir (R1) and the first element (1).
47. The breast implant system of any of claims 45 to 46, wherein the further reservoir (R1, R2) is adapted for implantation below the minor pectoralis muscle or between the major and the minor pectoralis muscles.
48. The breast implant system of any of claims 45 to 46, wherein the further reservoir (R1, R2) is adapted for implantation within the patient's body remote from the breast implant, the system further comprising at least one conduit between the remotely implantable further reservoir and the element or elements for fluid exchange between the further reservoir and the element or elements.
49. The breast implant system of any of claims 48, wherein the further reservoir (R1, R2) is adapted for implantation underneath the patient's arm.
50. The breast implant system of any of claims 1 to 45, wherein the breast implant (10) has a constant volume and a variable shape.
51. The breast implant system of any of claims 1 to 49, wherein the breast implant (10) has a variable volume and shape.
52. The breast implant system of any of claims 1 to 49, wherein the breast implant (10) has a variable volume and shape, but a constant mass.
53. The breast implant system of claim 52, comprising at least one chamber (14) forming part of the breast implant and containing a compressible fluid, wherein the first and second elements (1, 2) also forming part of the breast implant (10) contain an incompressible fluid, the arrangement being such that fluid exchange between the first and second elements (1, 2) results in a change of pressure within the at least one chamber (14), thereby causing a change of volume of the breast implant.
RELOCATION OF CUSHIONS
54. The breast implant system of any of claims 1 to 53, wherein the outer surface of the at least one first element (1) is displaceable relative to a contacting outer surface of the at least one second element (2) by moving the first element between different spots within the casing (3, 4) so as to change the outer shape of the breast implant (10).
55. The breast implant system of claim 54, wherein the second element (2) is stationary mounted.
56. The breast implant system of claim 55, wherein the second element (2) forms part of a wall (4) of the casing.
57. The breast implant system of claim 56, wherein both the first and second elements (1, 2) are movable within the casing (3, 4) between different spots.
58. The breast implant system of any of claims 56 to 57, wherein the first and
second elements (1, 2) are interconnected so as to limit their relative displaceability.
59. The breast implant system of any of claims 54 to 58, wherein either or both of the first and second elements (1, 2) are mounted so as to be movable only between predetermined spots.
60. The breast implant system of claim 54, wherein the first and second elements (1, 2) are freely movable within the casing.
61. The breast implant system of any of claims 54 to 60, wherein the at least one first element (1) is movable from a spot located at least partly beside the at least one second element (2) to a spot on top of the at least one second element so as to change the outer shape of the breast implant from relatively flat to relatively high.
RIGID BACK PLATE AND CASING
62. The breast implant system of any of claims 1 to 61, wherein the breast implant (10) has a rigid back wall (3) arranged to being placed adjacent the patient's thorax.
63. The breast implant system of claim 62, wherein the rigid back wall (3) forms part of the casing (3, 4).
64. The breast implant system of any of claims 1 to 63, wherein the casing (3,
has a structure comprising at least one compartment filled with a soft material, such as a foam or silicone.
SERVO SYSTEM
65. The breast implant system of any of claims 1 to 64, wherein the exchange of an amount of fluid between the reservoir and the casing (3, 4) involves a displacement of an amount of fluid between sub-chambers (R", R"`) of the reservoir, said amount being different to the amount of fluid exchanged between the casing (3, 4) and the reservoir.
66. The breast implant system of claim 65, wherein the exchange of an amount of fluid between the reservoir and the casing (3, 4) involves a displacement of an amount of fluid between the sub-chambers (R", R"`) of the reservoir which is less than the amount of fluid exchanged between the reservoir and the casing (3, 4), and vice versa.
67. The breast implant system of claim 66, wherein at least one (R`") of the sub- chambers of the reservoir is adapted to being implanted subcutaneously such that fluid can be exchanged between the sub-chambers (R", R"`) by manually compressing the subcutaneously implantable sub-chamber (R`").
68. The breast implant system of any of claims 1 to 67, wherein a spring element (25) is provided for urging the reservoir (R) or at least one (R') sub-
chamber thereof into a state of minimum or maximum volume.
INJECTION PORT FOR CALIBRATION OF VOLUME
69. The breast implant system of any of claims 1 to 68, comprising at least one injection port (31) adapted to be implanted under the patient's skin and
connected to any reservoir (R; R1, R2) so as to allow fluid to be added to or removed from said reservoir by injection from outside the patient's body.
70. The breast implant system of claim 69, wherein the system is adapted to maintain shape and size of the breast implant while fluid is added to or removed from said reservoir via said injection port.
PUMP
71. The breast implant system of any of claims 1 to 70, further comprising at least one pump (13).
72. The breast implant system of claim 71, wherein the at least one pump (13) is adapted for exchanging lubricating fluid between the reservoir (R) and the casing (3, 4).
73. The breast implant system of claim 71, wherein the at least one pump is adapted for pumping fluid between the first and second elements.
74. The breast implant system of claim 73, further comprising at least one further reservoir remote from the breast implant and connectable to the at least one pump so as to allow for fluid exchange at least between the first and second elements (1, 2) by pumping fluid with said pump from the first element into the further reservoir and from the further reservoir into the second element, and vice versa.
75. The breast implant system of any of claims 71 to 74, wherein the at least one pump is adapted for implantation under the patient's skin so as to be manually operable through the skin.
MOTOR FOR PUMP
76. The breast implant system of any of claims 71 to 75, further comprising at
least one motor (M) arranged for automatically driving the pump (13).
77. The breast implant system of claim 76, comprising a manually operable switch for activating the motor, the switch being arranged for subcutaneous implantation so as to be operable from outside the patient's body.
78. The breast implant system of any of claims 76 to 78, wherein the motor is arranged to be driven by electric or electromagnetic energy or by an electric or magnetic pulsating field or by ultrasonic energy.
ENERGY SOURCE
79. The breast implant system of any of claims 1 to 78, further comprising an energy source (E, 29, 30) for supplying energy directly or indirectly to at least one energy consuming part of the system, such as to a motor for driving a pump.
80. The breast implant system of claim 79, wherein said energy source includes
an energy storage means (E).
81. The breast implant system of claim 80, wherein said energy source includes
one or more of a battery, an accumulator, a rechargeable battery and a capacitor.
82. The breast implant system of any of claims 80 to 81, wherein the energy storage means (E) is adapted for being implanted inside the patient's body.
WIRELESS ENERGY TRANSMISSION
83. The breast implant system of any of claims 79 to 82, wherein the energy source comprises a wireless energy transmitter (29) adapted to wirelessly transmit energy from outside the patient's body directly or indirectly to the energy consuming part.
84. The breast implant system of claim 83, comprising an implantable energy transforming device (30) for transforming wirelessly transmitted energy into electric energy.
85. The breast implant system of claim 84, wherein the energy consuming part is driven with the electric energy, as said energy transforming device transforms the wireless energy into the electric energy.
86. The breast implant system of claim 83, wherein the energy consuming part is adapted to directly transform the wirelessly transmitted energy into kinetic energy.
CONTROL UNIT
87. The breast implant system of any of claims 71 to 86, further comprising a control unit (C1, C2) adapted to directly or indirectly control the at least one pump (13) or another component of the system.
88. The breast implant system of claim 87, wherein the control unit (C1, C2) is adapted to be operable by the patient from outside the patient's body.
89. The breast implant system of claim 88, comprising a manually operable switch for activating the control unit, the switch being arranged for subcutaneous implantation so as to be operable from outside the patient's body.
90. The breast implant system of any of claims 87 to 88, wherein the control unit comprises a first part (C1) adapted to cooperate with a second part (C2) from outside the patient's body and the second part (C2) is adapted for implantation in the patient's body.
91. The breast implant system of claim 90, wherein the first part (C1) of the control unit is adapted to wirelessly transmit a control signal to the implantable second part (C2) of the control unit for controlling the at least one pump (13) or other component of the system from outside the patient's body.
